Question: Do I need a doctor's order?
Answer: Yes, a physician's order is needed for therapy services.  When a physician prescribes therapy services, you can choose which outpatient therapy clinic you would like to receive your therapy. We have conveniently added a printable order for under our "Forms" tab that may assist you. When asking your doctor for a prescription, please be sure it says physical therapy "Eval and Treat," has your diagnosis, your doctor's signature, and the date on it. Your physical therapy must start within 30 days of that date in most states.
